When you join a UNC Wellness Center, you can choose from a variety of membership types, including:

Membership Documents

During the enrollment process, the primary member must complete a membership application while all members must complete the Acknowledgement of Receipt of NPP brochure, Waiver and Release of Liability, and the Physical Activity Readiness Questionnaire. When completing forms for your dependents (ages 0 – 17), please print the child’s name in the appropriate locations and include the parent or guardian’s signature in the appropriate locations.  Each dependent 18 – 24 years old must complete a set of forms for themselves.

Please schedule an appointment at your desired location to submit paperwork and complete the enrollment process.  A membership will be active once a membership representative has received all of the required paperwork including billing information and verified the information on the forms.  Please email us at wellness@unch.unc.edu to set an appointment with our membership office at either location to submit the necessary paperwork and complete the enrollment process.

30 Day Guarantee

The 30 Day Guarantee provides new Fitness, Corporate, and UNC Employee members with the opportunity to fully explore the membership option they have chosen, while still maintaining an option to terminate the membership within the first 30 days of their membership. Assuming appropriate notice and timing of cancellation during the first 30 days, the member will only be responsible for one full month of membership dues.

Additionally, all memberships are month-to-month with no long-term contracts. If the member decides at any time after the first month that it would be best to end the UNC Wellness Center membership, the member can do so by providing appropriate notice within 30 days.

Cancellations

Memberships are month-to-month and can be canceled at any time with appropriate notice.  To cancel, a member can notify the Membership Office to provide the required written notification.  The cancellation will go into effect once all documentation is completed and approved by appropriate UNC Wellness Center staff at least 30 days prior to the beginning of the next billing cycle. If the documentation is not completed and approved at least 30 days before the billing cycle, the cancellation may not take effect until the beginning of the following billing cycle.

UNC Wellness Center management reserves the right to terminate a membership for any reason. Management would typically make this determination only when membership dues are not paid or when member (or member family or guest) behavior is grossly or habitually in violation of UNC Wellness Center regulations.
